HSOP requires the following information from your organization in order for any animals to be transferred to your  organization's custody.  Your application can be completed and submitted below. 
 
If you prefer to complete the form manually, you can print a copy of the application by using the print button on the bottom of the form.  Please return the completed application and a copy of the adoption form your organization uses to us via fax, email (rescue@hsop.org), or regular mail.  Fax and mailing information can be found below.
